Sample Order Form

WebPathLab's on-line requisition allows the referring clinician to not only easily submit an order, but also the ability to track and data-mine all requisition information.  The on-line requisition accesses the patient's database so that in the event of having a returned patient, the system can autopopulate the patient's demographics. This is a time saver for the referring physician. 

We can match the "look and feel" of your current paper requisition form so that your customers will already be familiar with it

Once the order form has been completed, it is printed along with either an alphanumeric label, or a bar code (1-D, or a 2-D) label which is then attached to the specimen.  When the specimen(s) arrive at your lab, the information is already in the system.
